Build intake forms and waivers

Paper forms get lost; email threads scatter health history. Revively forms attach answers to the customer profile and booking.

Create a form template

1. Go to Settings → Forms and create a new template - intake questionnaire, health history, or liability waiver.
2. Add fields: text, yes/no, signatures, and required markers.
3. Save the template. Vertical packs may include starters you can edit.

Send before the visit

1. Attach the form to a service or send a link by SMS or email from the customer record.
2. The customer completes it on any phone - no app required.
3. Submissions appear on their profile and linked booking.

Review at check-in

Staff open the profile before the session to read responses and signed waivers. Update templates anytime; previously submitted answers stay on file for compliance. Pair with customers for a full client record.
